The Impact Assessment (IA) Form is the follow through form that aims to assess the knowledge, confidence, and understanding of learners of the basic concepts of cyberhygiene training after 3-6 months of training completion.

Kumusta? We hope that you are all well and protected against scams and cyber risks! It’s been a while since we had our cyberhygiene training. As mentioned during our training, we are back to check on you on your learning application of cyberhygiene concepts after our training.
Please follow these instructions for answering the form and for a chance to be part of the randomized draw of the impact assessment incentives!
- On the left side, there’s a link to the Impact Assessment Survey and it will lead you to SurveyCTO. Please select your preferred language in the upper right corner of the form. These languages are Filipino English, Tagalog, and Cebuano.
- After answering the Impact Assessment Survey, please screenshot the survey accomplishment confirmation and go back to this website.
- After this, please click the link on the right side that will lead you to the Impact Assessment Confirmation form via Cognito Forms. Please fill-in your details and upload the screenshot.
- Upon completion, you will receive an email and we will contact you if you are lucky to be part of the randomized draw of incentives.
